Dr. Matt Broghammer, a Winona Health general surgeon, will be dancing in the annual Dancing With Winona Stars fundraiser on behalf of Live Well Winona. Funds will go toward a new community health coach program called the Community Care Network (CCN). This collaboration between Winona State University and Winona Health trains students to help high-risk patients and community members follow a medical care plan by engaging them in their homes and keeping their hospital visits to a minimum.
“While the initial driving factor is to reduce Emergency Room visits and Hospital admissions, the ultimate goal is for a healthier population. The pilot program has shown incredible results of over a fifty percent reduction in visits/admissions,” says Dr. Broghammer.
